堪らん and variants of this in hentai are (usually) a guy saying he can't get enough of something (i.e. sex with the girl). I am guessing this is on it's own line, since it is related to the following text but it's its own sentence, basically.

まだ足りん is saying he hasn't had enough / is not satisfied yet (even though they've been doing it the whole day).

I think I got it.

The first one is mostly correct except for tense. The て in お漏らしまでしちゃって doesn't mean present tense, it's connecting to the second part of that sentence (so it's also past tense). So it should be some variant of "you wet yourself so..."

There's no "if" in the second sentence though. オムツオナニーであんなに激しくイケるの is all one thing (being able to cum that hard from masturbating in a diaper) and it's being marked with って because it belongs to the following part. Aないと（いけない） means "have to A" (the な after ないと is just an emphatic sentence ender). 時間を有効に使う means using time effectively. Time here would obviously be the time in which Mamoru (or however the name is read) isn't home. So in the first part, I'd assume he's saying "(we) have to use the time (that Mamoru isn't home) effectively...!" or just "we have to use this time effectively...!"

For the second part, presumably the husband and wife had a prior agreement (約束) which was that it was OK to do it so long as Mamoru wasn't able to see them doing it. (I'd infer that the wife had issues with doing it in the home they're all living in and agreed to do it only if they weren't going to be discovered by the younger brother.)
