"it's fine (even) if you cum", so basically 'you can cum (if you want)'.
it probably sounds more straightforward rearranged a bit: いってもいいよ
(which is what you get reading it left to right. I never saw left to right vertical writing but it could be a thing, who knows. it makes sense either way, since in japanese you can sometimes move things around and keep the same meaning)
