I was going to but i started doing work for a publisher, and i don't have time right now.

Hmm that seems hard and this is purely my guess, I've only read the previous page to see if anything helped with the context but no luck, but it seems he's referring that it would still be okay if she'd hit/slapped him (or probably his dick) sideways (horizontal motion) but not hitting him (his dick) vertically (motion).

If you're a guy and hanged around the stupid group in college you must've done (and consequently had this done to you) this before, hurts a lot, but since it doesn't hit your balls it just feels like burning and fades quickly (like a slap, but in a sensitive area), whereas hitting it "fully" (like in a vertical motion) in your balls feels like you won't ever walk straight again.

But well, I'm not entirely sure, this is purely my guess based on the sentence alone.

Huh, I didn't really consider that. I guess I see where you're coming from physiologically, but it still seems like a bit of a stretch...? Or is this some sort of really, REALLY odd Japanese saying regarding low-blows? I mean, this is the society that came up with 電気アンマ, after all...

I'm glad I'm not the only one who found this one tricky. Anyone else have any thoughts?

Noun + として indicates the capacity, role or function of that noun, usually translated "as":

私は医者としてあななに言います : I'll tell you this as a doctor
セールスマンとして採用された: he/I/she was hired as a salesman
この部屋は物置として使っている: I'm using this room as a storeroom

that part there is definitely "as your brother". I read 何かこう as "somehow"
a more literal interpretation would be " somehow, as your brother..." So i think Danicco is correct in his interpretation.

Sorry I was sorta distracted and forgot to explain that, but yeah I assumed that.
Since I don't think the sentence would make much sense in english with just "Something like this, as your older brother...", you'd usually "something like what?" right?

Then I went as "Something like what" -> "something like only me feeling good" -> "I should do something in return".

Am I even making sense? I don't think I am, but that's what I thought at the time.

I don't think so because of the 何かこう, if it was that I think he'd say これこう or こんなに (and wouldn't refer to himself as the 3rd person, and/or would put her as the subject of the sentence, among others) or something like that.
