Yep, they're brother and sister, but the girls aren't siblings.

Ah, sorry, my bad, I didn't realize just how little you could see there...........she's tied up spread-eagle and that's pointing at her foot/curled toes (?)

Well, I am going to make a guess here.

I am guessing she had shoes or something on up to that point, and then in this particular panel the artist suddenly drew her without shoes. When he realized, he put that note in, saying basically "she took them off, teheh" to jokingly explain the mid-scene inconsistency after the fact.

That's the most likely explanation I can come up with based on the placement (indicating an author's note) and the arrow (pointing at her foot). But without checking it against earlier scenes myself, I can't say for certain. It's also possibly referring to some other article of clothing and that's just the only out-of-the-way place he could fit it so it just looks like it's pointing right at her foot. And there's always the possibility that it's something more obscure that you wouldn't think of without seeing the rest of the scene.

I would like some help with this page.

I have translated it like this right now:
