1)Basically all equipment got levels now.
If you have any equipment from before the patch these are all "level unassigned" which will automatically be assigned to your current level up equip. But all new equipment you get from now on will be assigned the level you got them at.
The level on equipment determine the effective stat (note base stat does not change). Basically in order for equipment to have its effective stat scale with you level you need to soulfuse it (bind to your account forever). Soulfuse cost soul fragment which is gained by doing:
random encounters (best way averaging 6 per day)
buying it from shop at expensive 10k per piece (not worth it)
winning from lottery (will highly unlikely ever win it)
Soulfusing has set cost which you can read on the patch note 0.83.
But basically long story short if you don't soulfuse your equipment get weaker and weaker as you level up because they are stuck at the level you found them at, but if you soulfuse the equipment stat will scale with you.
2)Items now have CD of 40 turns from memory and can be used infinitely.
3)Yes random encounter occurs every 30 minutes

About my level it probably due to the patch making leveling easier. My lvling speed is actually quite slow since I was deliberately slowing my lvling due to the soulfusing equipment changes brought about by the new patches. I say this because I think I saw a guy who started later than me and is currently beyond lvl 350 now.
