Time. You can clear BT twice as fast. Worse XP, but better everything else.
This. When active I try to go for 4 hour stints with a bunch of shards activated. On Battletoads, that lets me clear all of the arenas up to The Trio and The Tree and still have time to spare for a 200+ level grind fest. On IWBTH, I imagine I'd get through less than half than that, so my net gain would be considerably lower.
IWBTH is a much better pay off per stamina, but Battletoads is a much better pay off for actual time. I'd only go with IWBTH as a standard if stamina was more of an issue than time, or if I could convince myself that the loot quality advantage was important enough.

Other suggestions?
The safe and easy way of doing them is by using the schoolgirls to fill your OC, then focus like crazy. Repeat ad nauseam. You should never run out of anything that way, even on IWBTH, it just takes ages. Hours on hours. As you repeat it you'll work out just how much OC you can spare on skills to speed the process up, but it's never fast. It's these time concerns and sheer boredom that mean that nearly no one does them on a regular basis.
In my opinion, 220+ should never willingly be approached as melee other than as DW, which can kill the schoolgirls way faster than 2H of any flavour. The difference is huge, and even then it takes too long to really be worth it beyond initial clearing. Your mace should still be able to do the job, but with lower raw damage and no PA it'll just take so long that I recommend you go for it with a DW set up unless your DW proficiency is non-existent.
Having said that, my DW set is pretty poor, so someone with a top class set might be able to confirm that it doesn't take hours with proper DW.
