Hard to say. Just because it's not the focus of the manga doesn't mean that none of the women are being made to bear the demon lord's children. The only thing we know from the events depicted is that the demon lord eats human semen and prefers(?) futanari women's semen.

Apart from the heroine who went to rescue the women in the cave, all of the futanaris in the cave were normal women who had their bodies modified to become futanari. As far as the heroine knows, her village is the only place where there are natural futanaris.

It looks like a magic spell (like "Abracadabra") that plays on チチ (breasts) and オッパイ (breasts) and possibly チンポ (cock).

チチンパイパイ is actually a play on "ちちんぷいぷい" a commonly used "magic word" for children.
