It's the former, "It got hard (yet) again", probably with a negative connotation (or a sweet one?). It's basically また勃って来た = became hard again, so the transition is already over.

There's an implied negative after the ellipsis. He's just saying he hasn't said anything (a word) about being willing to do it yet.

The tail is misleading. It doesn't make sense as something the man said after the previous panel and makes more sense as a continuation of the first (angry) outburst.
