Im not sure either.
But maybe its kind of exaggerated way of "liking". Usually there are wide range for different ways. For example, i like stocks. Not exactly in bdsm style, but sometimes it looks good. For example, Murakami quite often use that. So core is fastening and lighter version is something like handcuffs or band binding which like/tried almost everyone. But stocks or bondage is already quite specific stuff and its much heavier. And maybe... i dont know exact tag for that, but its amputated stuff. We can say its kind of fastening too, but in rly exaggerated/perverse way. So probably there are few ppl who can like even that.

And if we speak about liking as something like arousing/immersion then there is one perv thing which i sometimes find good. Its partially connected with ahegao, but i a bit different. It probably comes with some specific character traits. We can say vanilla is something about romantic person or maybe very respecting females. While hardcore usually means ego nature. So probably there is something what makes ppl find that appealing. I cant get that, so its hard to say. First, person should at least dont find that disgusting. I was learning anatomy and actually can prove ppl can get used to that. It takes huge amount of time and efforts, but can be done up to certain extend. But that doesnt makes such stuff appealing completely. It just doesnt trigger negative emotions so much. So person probably should have any kind of desire to watch that. Maybe some complex, maybe something else. And after that it ll be pushed further by the fact plot makes pleasure superior even up to such degree.
