"Costume tags [...] only apply if they are kept on (at least partially) during sexual activities."
In both cases, once the hat is removed, there isn't anything witch-like about the outfit. It's a standard outfit for the character, who is a witch, but the character doesn't wear typical witch clothing aside from the hat. So only the hat matches the description of the witch tag: "A tall pointed hat with a wide brim and usually cloth robes" (and the hat is the only absolute in the tag description anyway).
