9 3 Amphibious Sperm Whale hits you for 42 crushing damage.
9 2 You gain the effect Regeneration.
9 1 You use Health Gem.
I just had the same thing happen:
14 4 Your spike shield hits Scary Ghost for 8 points of cold damage.
14 3 Scary Ghost hits you for 58 physical damage.
14 2 You gain the effect Frost Spikes.
14 1 You use Frost Gem.
Since gems are supposed to have a usage time of 0, there's no way that the Ghost should have had time to attack me before I took my next action.

Usage time of 0 on gems is definitely busted.
