You get points with every purchase, which can then be used to discount following purchases.
DLsite used to have paypal, but not sure if it still does on the adult side of it without checking.
Edit: Just checked, they use paypal still, but it's now disabled on adult products.

I do know you can use a debit card, I've done this before.

what you actually need to do is find a site that sells something called "bitcash". It's pretty much a coupon code that only exists in japan and can be used on various websites, including NFSW DLsite. I think playasia used to have them. Anyway, you get an 18 or so digit hiragana code you can use on DLsite to charge up your points, allowing you to effectively buy shit through paypal.
