My biggest fuck up: not keeping track of who gave me what as a present.
I don't remember who gave me my first Exq Power set, my first dual Superior Ethereal Shortsword set, who (aside Jenga) gave me a few miscellaneous presents as equipments, credits or other items.
I still do remember some of those though.

My biggest regret is having soulfused my light armor type equipment too early.
Didn't have/know of IW service at the time, else I would have at least asked for IW10 on everything before to make it a bit better, especially since it's almost impossible to upgrade it for reasonable cost due to it being full shade.
Well for now, I'm using clothes since I play mage, but if for some reason I temporarily went back to 1H/Light, well the regret would come knocking back.

I've done the "oops, didn't notice it was still high-set difficulty in TT&tT" bit.

Also, I'd soulbind everything. 90% of my play is random encounters because they're the best XP for time investment, and so I have beaucoup soul fragments and not much else to do with them, and I figured "hey, I'm gonna item world these things eventually" or at least "I'll let its stats scale while I wait for better equipment."

One of my biggest mistakes is trying to scan a legendary enemy in arena to find its weakness, but miss the "scan" button and hit the "flee" button instead.

I was so close to winning, but accidently left the arena challenge because of a stupid mistake.
