How could you inflict imperil without casting it?

I think it doubled up some of the lines. You can see I somehow got the exact same spell damage twice in a row (and also cast Chained Lightning twice in a row, which also isn't possible), so it was probably Imperil - Chained Lightning, and it doubled up starting from the Imperil spell effect.

Actually, I never bought them. I was around level 350, magnificent (and possibly exquisite) equipment, and playing mainly random encounters when soul fragments were added. By the time I had something that I wanted to soulfuse I had more than enough fragments.

I have "legendary" and magnificent gear, am level 219, and have two monsters, and have been on e-h since 2012, but just now realized I'd never upgraded a single piece of my equipment. Now it suddenly makes sense why i've made so little progress recently (and why battles take so long).
Might still be a good idea to switch to a heavier-than-cloth armor, too, since I don't find myself using offensive spells too often either anymore. Just recovery stuff, protect, regen, shadow veil, that sort of thing really. I dual-wield a shortsword and a rapier so I already am pretty low defense.
I'm a special kind of stupid.

I was never any good at runescape either.
