You can have people getting raped, killed, tied up, getting there body part's cut off, have there head on a pike and all that, but if there is JUST ONE tiny bit of a penis, OH NO people get offended.

I can think of a few reasons why that is.

1. Most gamers are guys and most guys don't want to see a dick.
2. Most girl gamers are bisexual and don't mind seeing naked women. Hell, most straight women don't mind because women aren't insecure about their sexuality like men are.
3. Dicks are more subjective, vulgar and offensive than pussies and they're the main body part attributed to porn. They come in different sizes and have two states, erect and flaccid. What kind would they show in a video game? A big hard one? That would be an insult to so many people. A pussy though, they mostly look the same, especially in mainstream stuff because the chick almost always has a bush.

I guess a more specific example would be Oblivion's ESRB re-rating:
It went from Teen to Mature due to "blood and gore" and "sexually explicit content". I agree on the changes due to discovering far more "blood and gore" than when originally rating it.
The "sexually explicit content" was from discovering they had kept the texture for the topless females mode, which was only accessible trough 3rd-party mods.
