Thousand Arms for the PS1, Atelier Iris series for the PS2, Gihren's Greed S-RPG Gundam games and Valkyrie Profile 2 for the PS2, and last but not least Cross Edge for the PS3.

A game that, in my eyes, deserves much more attention, is Beat Hazard, an arcade shooter, where more or less everything is based on the music you are "playing".
Three other games that are'nt well known, are Submarine Titans and Aquanox 1&2. Correct me if I'm wrong, but they seem to be rather unknown...

Spiderman 1 and 2 on PS1. It made a bit of a buzz when it came out but everyone stopped caring when the movies came.

Actually, Spider-Man 2: Enter Electro was a complete disappointment in practically everyone's eyes. After all, Neversoft didn't make it.
Monster Lab for the Wii. The game looks like shovelware but it's actually a really fun RPG title.

if you like classic style RPG, you should try Eschalon Book 1 & Book 2 both offers you traditional RPG style with modern enchantments. it's use turn based system + hack n slash style.

Golden Sun and Golden Sun:The Lost Ages for GBA. The character development is almost non-existent, but for some reason, I just had so much fun playing the game. I recognize that it's not a game for everyone.
Anyways, after about 7 years, they finally decide to make a sequel for the DS, Golden Sun: Dark Dawn. If you like RPGs, then I suggest you check it out and see if it's your cup of tea. If you don't mind playing teh old games, try out the ones for GBA as well.

uh, golden sun are totally NOT hidden gems. neither ico nor shadow of the colossus.
a hidden gem... mmm i would have to say skygunner for ps2. its a short yet addictive arcade flight simulator that passed largely unknown.
another very personal gem, and that nobody seems to know about it, is heavenly guardian for the wii. this game is a spiritual succesor to pocky and rocky, as the game plays the same. it has SNES graphics, snes durability, and nes difficulty.

Good thing is there is going to be a re-release of Ico and Shadow of the Colossus in the early part of next year on the PS3. I played them but never bought them back on the PS2 but I'll be sure to buy them this time around. Besides, an improvement on the frame rate on Shadow of the Colossus will definitely be welcomed.

I might get it if there's a decent graphical improvement, but maybe even if there's not. Only played the ico demo. Didn't think too much of it myself. Didn't have SotC's grandness. I'd be playing the latter right now if my shitty PS3 was backward compatable. Instead of backward assed.
